GP workload crisis, vaccination passports and the PCN DES 21/22

health No Comments »

Join Andy and Gandhi for our Digital Primary Care update including discussing the dawn of a GP workload crisis, vaccination passports, PCN DES contract and more….

Is there still a workload crisis?

#GPworkload #workloadcrisis #vaccinationpassports

00:00 Welcome

The last few weeks is it busy?
Andy / Gandhi

The theme in the industry media around workload

Year-end for Practices and PCNs, looking forward to 21-22

Changes to workload pressures QOF restart
Weโ€™re looking at some stories and tell you about content coming up in next few weeks…

03:30 GPs working average 11-hour day, major survey reveals

https://www.pulsetoday.co.uk/news/workload/gps-working-average-11-hour-day-major-survey-reveals/

11 Hour WW notes
Results from Pulse workload survey – 1,400 GP respondents
Look at the infographic and discuss
11 hours working day and 37 patient a day (28 thought to be safe)
Longer consultation times

Survey also asked about the impact of the pandemic
See infographic and discuss

Are telephone and online consultations saving time?
More stressful, less information (body language etc.), risk of f2f in a pandemic to manage

What can we do?
Ermโ€ฆ.. Difficult
Recognise the impact of technology – pandemic safety and patient convenience yes, helping doctors – no?
Restart some old discussions about funding models, presentation of availability by gov/nhs, managing demand, token chargesโ€ฆโ€ฆ?????

18:20 BMA says GPs can refrain from non-essential work despite April restart orders
https://www.pulsetoday.co.uk/news/workload/bma-says-gps-can-refrain-from-non-essential-work-despite-april-restart-orders/

Notes
Ready to restart QOF activity in April, CQC resuming some inspection activity
But joint BMA.RCGP guidance – January –
But ongoing Vaccination clinics, maintaining social distancingโ€ฆ which says โ€˜mostโ€™ GP practices should pause all or a โ€˜significant proportion of their non-essential work, depending on local Covid rates โ€“ โ€˜remains relevant as we continue in the pandemic situation
Ongoing discussions about pausing QOF again???? – BMA spokesman
? non-story ish?

Workload impact of vaccine passports?

21:30 GPs should have no role in administering any vaccine passport scheme, RCGP warns

https://www.pulsetoday.co.uk/news/clinical-areas/infectious-diseases/gps-should-have-no-role-in-administering-any-vaccine-passport-scheme-rcgp-warns/

Notes
Government is likely to make some activities dependant on vaccination status – pubs, holidays abroadsโ€ฆ.
RCGP getting ahead of this and stating scheme should have zero impact on GP workload

Episode coming up about Vaccine passports when we know more in the coming weeks

Just use the NHS App as the basis for โ€œpassportsโ€? Tab on their confirming vaccine status with authenticating QRcode?….Just call it allowing people to demonstrate vaccination statusโ€ฆ legislate for misuse/discrimination and let industries decide.

Sex Determination: Who is responsible for the gender of a new born baby?

sex No Comments »

Welcome to our informative video where we delve into the fascinating world of chromosomes and unravel the mystery behind determining the sex of a child. Discover the fundamental building blocks of life – cells, which contain thread-like structures called chromosomes. Gain insights into human chromosomes, where each cell contains 46 chromosomes organized in 23 pairs. Explore the unique nature of sex chromosomes, the one pair that differs between males and females. Learn about the two types of sex chromosomes, namely X and Y, and their presence in male and female cells. Understand the crucial role of reproductive cells – sperm and eggs – which carry only 23 chromosomes, one from each pair. Witness the process of fertilization, where the combination of a sperm cell and an egg cell forms a zygote with 46 chromosomes, ultimately developing into a baby. Uncover the connection between the presence of certain sex chromosomes and the determination of a child’s sex. Expand your knowledge on the influence of male reproductive cells in determining the gender of a child. Watch this video to gain a deeper understanding of chromosomes and the fascinating world of human reproduction.

chromosomes and cells, sex chromosomes, X and Y chromosomes, reproductive cells, sperm and eggs, zygote development, determining child’s sex, male and female chromosomes, sex determination in humans, genetic inheritance, human reproduction, understanding chromosomes, child gender determination, biology of reproduction.Every organism is made from cells. Cells contain thread like structure known as chromosomes.

In humans, there are 46 chromosomes in each cell. And these chromosomes are found in pairs. So there are 23 pairs.

In male and female, all the pairs of chromosomes are same except one pair.
Chromosomes in this pair are called sex chromosomes.
Sex chromosomes can be of 2 types namely X and Y. In a male cell, both x and y chromosomes are present but in female cell 2 x chromosomes are present.

We know that male reproductive cells are known as sperm and female reproductive cells are known as eggs. reproductive cells are different from normal cells as they contain only 23 chromosomes. One from each pair.

When sperm cell and egg are combined together, they form a cell which has 46 chromosomes or 23 pairs. This new cell formed is called zygote and develops into baby.

Since reproductive cells have only 23 chromosomes, sperm cell can contain either x or y chromosome. And a egg cell will always have x chromosome. During reproduction, lot of sperm cells are released and only one sperm fertilises the egg.

when sperm containing Y chromosome fertilises the egg, resultant cell will have a sex chromosome containing x and y chromosome and the zygote will grow into a boy.

But when the sperm cell containing x chromosome fertilises the egg, resultant cell will have a pair of x chromosomes and the zygote will grow into a girl.

It is clear that a child will be a boy or girl , depends on male and not the female.

The sex of the baby depends on which chromosome he or she receives from the father. X chromosome from dad will result in a girl, Y chromosome from dad will give a boy. Both are of equal value, to be loved and treasured.

Myth 3 – Is sex painful the first time?

sex No Comments »

When a woman has vaginal sex for the first time, it can be a little painful.ย There may also be a small amount of blood, but this isn’t always the case and usually occurs because the hymen has been broken during sexual intercourse.
The hymen is a small piece of thin skin (membrane) that can either partially or totally cover the entrance to the vagina. A woman might already have broken her hymen without knowing about it; this can happen whenย playing sports or using a tampon.
When a man has sex for the first time it shouldn’t hurt, but he can make it easier for his partner by using foreplay, making sure there is plenty of lubrication and by being gentle andย going slowly.

Let us know what you think in the comments below!ย

Her Body sheds light on a variety of health issues that affect women and teens including sex education, dating and relationships, and reproductive health.

Giving the Disease Less Power

TransCelerate BioPharma Inc., a non-profit organization dedicated to improving the health of people around the world by simplifying and accelerating research and development of innovative new therapies, has unveiled a new global campaign designed to help increase awareness of the growing need for clinical trial participants. The campaign, called โ€œOne Person Closer,โ€ focuses on inspiring healthcare professionals (HCPs) to consider clinical research as a possibility for their patients, incorporate the topic into their daily practices, and have meaningful conversations with patients about opportunities to participate in clinical trials.
One Person Closer is designed to put a human face on research and development by sharing the personal stories of researchers, HCPs and patients who have contributed to clinical research and the development of medical breakthroughs. The photojournalist campaign aims to show doctors, nurses and other HCPs that they can help facilitate research that advances science and that may benefit their patients.
